We know that for some members the process of enrolling and managing a healthcare plan can be intimidating. Having the right guidance and information can ease the burden and help you; your family and your healthcare provider make the right decisions. This member’s section is designed to provide you with easy to use, searchable information that other members have found helpful, as they plan for years of healthy living. As a member of the CPHL family, we want to make your healthcare experience as beneficial as possible. By enrolling in our Medicare Advantage Plan which includes prescription drug coverage, you can combine all your Medicare coverage into one convenient plan with no monthly premium, provided you are receiving both Part A and Part B coverage. As part of this plan, you must maintain your Part A and B coverage and continue to contribute to any cost shares associated with maintaining your current coverage.

Eligibility Requirements:
- You have both Medicare Part A and Medicare Part B
- You live in our geographic service area
- You are a United States citizen or are lawfully present in the United States.

- $0 Monthly Premiums
- $0 Deductible
- $50 every month OTC Benefit Card. OTC cannot be carried over from month to month.

Medication Therapy Management (MTM)
If you have complex health needs, you may be able to participate in a Medication Therapy Management (MTM) program. MTM is a service offered by Centers Plan for Healthy Living at no additional cost to you! This program helps you and your doctor make sure that your medications are working well. Medicare Prescription Payment Plan
Medicare Prescription Payment Plan (MPPP)
The Medicare Prescription Payment Plan is a voluntary payment option that works with your current drug coverage to help you manage your out-of-pocket Medicare Part D drug costs by spreading them across the calendar year (January-December). This payment option might help you manage your expenses, but it doesn’t save you money or lower your drug costs.
This payment option might not be the best choice for you if you get help paying for your prescription drug costs through programs like Extra Help from Medicare or EPIC (New York’s State Pharmaceutical Assistance Program/SPAP for short). Call us at 1-888-807-5717 (TTY 711), 24 hours a day, 7 days a week for more information.
