Welcome to Centers Plan for Medicaid Advantage Plus (HMO D-SNP)!
Centers Plan for Medicaid Advantage Plus brings your Medicare benefits, Medicaid benefits, and long-term care services together under one plan. This plan was designed for people with Medicare and full Medicaid who are age 18 or older, and need, or are expected to need, at least 120 days of community-based long-term care services. Centers Plan for Medicaid Advantage Plus allows you to remain in your home and community, receiving the services you need to maintain your health and safety.
As a member, you will be assigned a Nurse Care Manager who is a licensed professional who will work with you to help you:
- Manage your medical and long-term care needs;
- Work with your primary care physician to obtain needed services:
- Identify your goals;
- Create a plan of care to achieve those goals; and
- Help coordinate all your health services.
Centers Plan recognizes that the more information we provide, the better informed you, your family and your physicians will be to make the right decisions regarding your healthcare.
