Requisitos de elegibilidad/Puntos importantes de los Planes Institucionales de Necesidades Especiales (Institutional Special Needs Plans, ISNP)

Requisitos de elegibilidad:

  • Debe estar inscrito en la Parte A y la Parte B de Medicare
  • Debe vivir en un centro de enfermería (durante al menos 90 días)
  • Debe vivir en nuestra área de servicio contratada (Bronx, Erie, Kings, New York, Niagara, Queens, Richmond y los condados de Rockland)

Puntos importantes:

  • Prima mensual de $0 para la Parte A y la Parte B
  • Gestor de Atención Personal
  • Cobertura de medicamentos recetados
