  • Centers Plan for FIDA Care Complete (Medicare-Medicaid Plan) is a managed care plan that contracts with both Medicare and the New York State Department of Health (Medicaid) to provide benefits of both programs to Participants through the Fully Integrated Duals Advantage (FIDA) demonstration.

Who is eligible for Centers Plan for FIDA Care Complete?

    • Residents of the 5 counties listed above,
    • Who are 21 years old, or older, at the time of their enrollment,
    • Who are entitled to benefits under Medicare Part A and enrolled under Medicare Parts B and D, and receiving full Medicaid benefits;
    • And who also meet one of the following:
      • A Nursing Facility Clinically Eligible individual who is receiving facility-based long-term supports (LTSS);
      • An individual who is eligible for the Nursing Home Transition & Diversion (NHTD) 1915 (c) waiver; or
      • An individual who requires community-based long-term care services for more than 120 days, as determined by a State approved clinical assessment.
